Las Vegas, NV (KTNV) -- NV Energy has been cited and fined following an investigation into the death of a worker in Las Vegas last fall.
The worker died after falling 75 feet while working on a power line on September 25.
He was later identified as 29-year-old Herbie Dempsey Goforth III.
The Occupational Safety and Health Administration launched an investigation into the death, finding seven safety and health violations.
One of the safety violations was protective equipment with holes and tears.
Investigates said NV Energy used equipment like ladders and platforms improperly.
OSHA proposed a penalty of $43,000 for the violation.
NV Energy now has 15 business days to comply with the fines, request a conference with OSHA or contest the findings before a review commission.
Officials with NV Energy issued a statement in response to the allegations, reading in part:
"NV Energy disagrees strongly with certain allegations in the OSHA citation and characterizations in its press statement. We intend to resolve our disagreements through the regulatory process..."
A spokesperson for NV Energy said the safety of employees is of "paramount concern."
